In more than 1500 patients from 13 European countries, Pons et al. demonstrated that secukinumab retention rates after four years were approximately 50% in both axSpA and PsA patients. Pons et al. aimed to assess retention rates and proportions of patients achieving remission and LDA, according to disease activity measures and patient-reported outcomes at 24 and 48 months, in axSpA and PsA patients initiating secukinumab. In this large real-world study, Pons et al., for the first time, report 48-month retention rates as well as rates of remission and LDA. Importantly, b/tsDMARD naïve patients demonstrated higher retention, remission and LDA rates than patients with prior b/tsDMARDs exposure, particularly in axSpA.  

Maksymowych et al. evaluated the effect of ixekizumab and adalimumab versus placebo over 52 weeks on structural lesions in sacroiliac joints assessed by MRI in patients naive to biological DMARDs with radiographic axSpA from the COAST-V study. The authors reported a decrease in erosion and increase in backfill at Week 16 with further reductions in erosion and increases in backfill occurring at Week 52 in patients receiving ixekizumab.

Østergaard et al. conducted a phase 4 multicentre, single-arm, open-label study to evaluate the effect of apremilast on MRI-assessed inflammation in PsA patients using PsAMRIS and MRI-WIPE. The study demonstrated that apremilast reduced inflammation in joints and entheses with no structural damage progression. The study also supports the use of MRI as an objective tool in PsA trials.

This study by Rech, et al. shows that 6-month treatment with abatacept was associated with a decrease in MRI inflammation, clinical symptoms, and risk of RA development in participants at high risk. The effects of the intervention persist through a 1-year drug-free observation phase.

Secukinumab efficacy regarding PROs and retention rate was comparable between axSpA and PsA patient groups when adjusted for confounders. Christiansen et al compared 6-, 12- and 24-month pain, fatigue, PGA, and HAQ PROs in axSpA and PsA patients treated with secukinumab, as well as 24-monthy retention rates in this real-world study.  

Comparing the alignment of European national recommendations for patients with PsA and axSpA with EULAR and ASAS-EULAR treatment recommendations found that only a minority of national treatment recommendations were completely aligned.

Tofacitinib treatment is associated with a significant improvement in CANDEN MRI scores of spinal inflammation in axSpA patients. This study by Østergaard, et al. also validates the CANDEN MRI scoring system as an approach for measuring axSpA-associated inflammatory lesions.

Investigators from a phase 2 study concluded that further investigation with BMS-986142 (a novel BTK) in people with RA is not necessary.

In this study abatacept 30/10 mg/kg and 10 mg/kg doses were associated with greater improvements from baseline in MRI detected synovitis and tenosynovitis, respectively, compared with placebo, confirming the clinical outcomes. Indeed, inflammatory components demonstrated the greatest change from baseline.

Maksymowych et al., assessed the effects of filgotinib on inflammatory and structural changes at various spinal locations, based on MRI measures in patients with active ankylosing spondylitis in the TORTUGA trial. Correlations between changes in the inflammation score and clinical outcomes were also evaluated.
