Recent MOA safety data emphasised the importance of patient-specific benefit-risk profiling in JAKi therapy, and extra-musculoskeletal (MSK) manifestations related to PsA should be considered during drug selection.

This study by Rech, et al. shows that 6-month treatment with abatacept was associated with a decrease in MRI inflammation, clinical symptoms, and risk of RA development in participants at high risk. The effects of the intervention persist through a 1-year drug-free observation phase.

Guselkumab treatment exhibited generally comparable and significant pharmacodynamic effects on IL-23/Th17–associated cytokines across participants with PsA who are biologic-naïve or have TNFi-IR. In coming to this conclusion, investigators assessed and compared immunologic differences and associations with clinical response to guselkumab in participants with active PsA who were biologic-naïve or TNFi-IR.

This Phase 3 RCT by D’Agostino, et al. assessed the long-term effect of secukinumab to placebo at tissue level on synovitis and enthesitis, and across all PsA manifestations. They found consistent improvements in clinically and ultrasound-assessed synovitis and enthesitis.

The results from Simon, et al. show that baricitinib treatment correlates with improvements in bone stiffness. Further improvements were also observed at the end of Week 52, with an increase in estimated failure load and no measurable progression in bone erosion being reported.

Post hoc analysis of guselkumab, Phase 3 DISCOVER-1 and -2 studies finds that 75% of guselkumab-randomised patients have complete resolution of dactylitis through one year.

Results from the 52-week phase 3 EXCEED study showed that secukinumab and adalimumab both display similar efficacy in time to resolution of enthesitis, in patients with PsA, irrespective of baseline enthesitis severity and individual site distribution.

D'Agostino, et al. aimed to evaluate whether treatment with secukinumab inhibits synovitis in patients with active PsA, as measured by PDUS. They found that secukinumab rapidly and significantly decreased synovitis, indicating a direct effect of IL-17 inhibition on the synovium in patients with PsA.
